PCMH Information
How to Contact your Medical Home
A Patient-Centered Medical Home provides increased access to care through:
  • Same Day Appointments
  • Extended Hours
  • After-Hours Resources
    • Call 330-499-3377 to discuss your care needs during the day and after hours. Our extended hours are appointments offered outside of our general appointment times and require prior approval from the practice. Appointments after 5:00pm are charged an after hours fee of $20.
    • After-hours calls, your care is supported by our answering service, triage nurses and on-call providers; call 330-499-3377 for these needs.
    • Please call the office first 24/7 for non-urgent concerns. For any urgent concerns contact 911 or go directly to the closest Emergency Department.
    • Our web-based Patient Portal also allows you to electronically communicate with your team and makes it easier and quicker to get the care you need.
Care Coordination
We want to assure that your care is coordinated
  • Tell us your medical AND family history, including symptoms and other important information. This may include past illnesses and surgeries, medications you are taking, etc.
  • Tell us if you have been to the emergency room or were admitted to the hospital so your care team can follow-up on your care.
  • Your care team assures your testing results are received in a timely manner to plan your treatment goals. Based on those results, your PCMH organizes all the medications you take.
  • We want to make sure the whole team is in the loop regarding your treatment.
    • Let your primary care Doctor know if you've been seen by another healthcare provider.
    • Let your specialist know who your primary care Doctor is so that he/she can follow up.
Partnering
As a partner on the team, you will need to:
  • Ask questions about your health.
  • Work with your team to set goals.
  • Share you concerns about reaching those goals so the team can help figure out the best plan for success.
  • Share your successes and challenges with your healthcare team.
Your Healthcare Team
A Patient-Centered Medical Home provides a structured team and coordinated communication. Your healthcare team works to help you feel your best by:
  • Listening to your concerns.
  • Educating you on how to manage all of your healthcare needs and empower you on how to take an active part in your health.
  • Addressing your healthcare needs based on the most recent research and recommendation.
  • Working with other Doctors and healthcare professionals to coordinate your care, including behavioral health care providers.
  • Give us your feedback so the team can continue to improve your quality of care.
Information When You Want It
A Patient Portal is a gateway to your health information and a way to communicate with your healthcare team. It allows you to:
  • Access your personal health record.
  • View test results.
  • Request medication refills.
  • Send and receive non-urgent messages to/from your healthcare team.
  • Keep up-to-date, sending you reminders when you need care or testing.
  • View patient education information.
